🩺 COVID‑19 Waiver | Head Above The Rim

Protecting Our Community: Your Health Matters

Participation in in‑person events, clinics, training sessions, and showcases involves physical contact and shared equipment. For the safety of athletes, families, coaches, and staff, all participants must complete this COVID‑19 Waiver before attending any on-site activity.

1. Participant Health Acknowledgement

By signing this waiver, I affirm the following:

  • I do not currently exhibit symptoms of COVID‑19 (e.g., fever, cough, shortness of breath, loss of taste or smell).

  • I have not tested positive for COVID‑19 within the past 14 days.

  • I have not knowingly been exposed to anyone with COVID‑19 symptoms or a confirmed case in the past 14 days.

  • I understand that COVID‑19 is contagious and participating in group sports increases exposure risk.

2. Assumption of Risk & Release of Liability

  • I acknowledge that Head Above The Rim takes preventive measures—such as sanitization, physical distancing, and personal protective equipment—but can’t guarantee complete protection.

  • Inherent risks of exposure exist in indoor and outdoor group activities, and I voluntarily assume all such risks related to COVID‑19.

  • In consideration for participation, I hereby release and hold harmless Head Above The Rim, its employees, volunteers, staff, affiliates, and facility operators from any and all claims related to COVID‑19 exposure, illness, or injury.

3. Shared Responsibility & Conduct

  • I agree to follow all health and safety protocols implemented by Head Above The Rim, including temperature screenings, sanitation stations, and restricted contact drills.

  • I will comply with any requests to wear face coverings, maintain physical distance, or modify drills as needed.

  • I agree to notify HATR immediately if I develop symptoms, receive a positive test, or learn of a close contact prior to any scheduled event.

4. Parent / Guardian Consent (If Under 18)

As the parent or legal guardian of the named minor:

  • I confirm the youth does not have COVID‑19 symptoms, exposure, or a confirmed diagnosis.

  • I consent to their participation and agree to all terms of this waiver on their behalf.

  • I affirm that the youth will follow all protocols introduced by Head Above The Rim.

5. Duration & Validity

  • This waiver applies per training season or per event series, based on Head Above The Rim protocols.

  • Continued participation requires renewal at each new program cycle or after any extended absence.
